Patient with overall beautiful nose whose desires changes along the bridge. She is concerned with the bridge width and the bridge discrepancy (the bridge is wider along the bones and narrower along the middle vault). After discussing the patient’s concerns, a closed rhinoplasty was performed and the upper portion was made to flow better into the middle portion of the nose. One advantage of the closed approach is that patients can maintain portions of their nose they like while changing portions of the nose they do not like. Closed rhinoplasty, particularly in this patient, is difficult as the contouring of the nose must be made appropriate.
