This patient was seeking a more masculine appearing jawline, decreased submental fat, and a subtle improvement in the appearance of his nose. The jaw is underprojected in relation to the rest of his face, as demonstrated by the faint red line drawn from his lower lip.
This patient also has increased submental fullness from excess fatty tissue underneath his chin. Fatty deposits beneath the chin may persist despite exercise, making surgery an option. A combination of direct excision of fatty tissue with liposuction to contour the neck results in improvement of his submental fullness and creates the illusion of a longer neck.
The chin was projected with a custom carved Gore-tex implant. The standard implants which are prefabricated serve as a rough guide to the chin shape and dimension. However, most chins are naturally slightly asymmetric and require subtle carving to restore natural symmetry to the jawline.
The nose in this patient is slightly underprojected and has a slightly acute nasolabial angle. A subtle increase in patient's projection and increase in his rotation by an open approach, provides the patient a natural, masculine appearing nose.
Overall, the patient's face looks more masculine and chiseled, yet with still a natural appearing face.
