Contracted Nose
Sometimes after rhinoplasty patients do not receive an optimal outcome and require a major reconstructive revision effort. In this example, the patient had 3 prior surgeries before seeing Dr. Shah. Originally, this patient suffered damage to her nose after a car accident. One of the surgeons placed an artificial implant in her nose (Gore-Tex). This implant became infected 6 years later and resulted in extrusion from the soft tissue envelope of the nose. One of the reasons Dr. Shah does not use artificial implants is that if they do become infected it can result in serious deformity of the nose.
This patient found Dr. Shah by direct referral from one of the most prominent and respected surgeons in the United States. She had met with nearly 15 surgeons, all of which could not or would not do her case. She met with Dr. Shah and he explained the limitations of her surgery. Her nose had such significant scarring and was so stiff that it was not possible to mobilize nose. Dr. Shah had the patient stretch her nose for 8 months prior to surgery in order to facilitate changes to her nose. Once Dr. Shah felt her nose was ready, he reconstructed her nose with costal cartilage.
On the postoperative views, the patient demonstrates improvement from her previous deformity. In these views her nose is 2 years out from her surgery with Dr. Shah. Major revision cases will often necessitate the use of costal cartilage due to the significant soft tissue scarring present.
